Step 2 - Fill out the form below to obtain a copy of J.A.M.'s 'Million-Dollar Insurance' policy to provide to the school of your choosing.
Step 3 - J.A.M. does not provide the funds for practice times, but most teams divvy the price among the parents. The usual rate being a few dollars per player.
